Patients often wonder if they are a candidate for liposuction of the abdomen or if they may require a tummy tuck (abdominoplasty). It is certainly a wonderful question and the right answer relies on the correct diagnosis to achieve the best possible result for rejuvenation of the belly and waist.

Simply Lipo

In general, women or men who have a limited amount of excess fat on the belly and waist and have great skin tone as well as muscle tone are the best candidates for liposuction alone.

Men who have not had a history of extreme weight changes can usually get away with liposuction alone.

Women who have never had children and are in overall good shape but have isolated areas of fat are often ideal patients for liposuction alone.

Some women even after pregnancy are fortunate enough to have fabulous skin and muscle tone for liposuction alone.

With either gender, the skin must have adequate elasticity to ‘shrink’ smoothly after removing fat without any excision of skin.

Trimming the Fat

Problems can occur with liposuction when poor skin tone exists. In cases where skin laxity is present, a tummy tuck may be required to remove the residual loose skin and tighten the abdomen.

Common reasons for severe abdominal skin laxity include changes following pregnancy as well as following extreme weight loss. If the skin laxity is present both above and below the umbilicus (belly button), a “full” abdominoplasty is required, where the incision is made from hip bone to hip bone and around the umbilicus. This will tighten skin that is loose all the way from the border of the ribs down to the pubic region. However, if loose skin is present purely below the umbilicus, a shorter incision can be used to remove a little loose skin just above the pubic region (often referred to as a “mini” tummy tuck).

In addition to fatty excess and skin laxity, many women following childbirth will develop “rectus diastasis” which is a separation of the stomach muscle edges in the midline. Muscle repair with sutures can be performed during a tummy tuck, which helps further flatten the abdomen and narrow the waist.

“The cosmetic surgeon should be consulted right away, prior to any other surgery, and it should be joint effort between the plastic or cosmetic surgeon and the breast surgeon,” says Michael S. Kluska, DO, FAACS, FACOS, a board-certified plastic and cosmetic surgeon in Greensburg, Penn.

The cosmetic surgeon’s role is not only to help you understand your reconstructive options, but also to consult with the surgeon who will be doing your breast cancer surgery.

“We can then collaborate on technique, and let the surgeon know what options are available,” from a cosmetic standpoint, Dr. Kluska says. “These options could include using your own tissue, or using some type of implant for reconstruction, whether that’s a tissue expander or just a saline or silicone implant immediately at the time of mastectomy.”

This is important, says Dr. Kluska, because how that surgeon approaches your breast with the first cut influences your potential cosmetic outcome. So choose your cancer surgeon carefully, too, he advises. “You need to seek out multiple opinions. Don’t stop at just the general or breast surgeon. Seek out a cosmetic or plastic surgeon who has experience in reconstruction in order to get an opinion. His or her role is not only to educate you as the patient,” but to help plan the placement of incisions for optimum treatment of the cancer allow for the best cosmetic reconstruction.

Sharon Y. Giese, MD, a board-certified plastic surgeon practicing and teaching in New York, agrees. Involving the cosmetic surgeon from the very beginning “gives women hope at the end of the tunnel—that they can have a nice looking (sometimes better looking) breast after a mastectomy or lumpectomy. Sometimes a lumpectomy can be coordinated with a breast reduction/lift surgery.”

And sometimes cosmetic surgeons can perform a breast reduction or reconstruction with an implant at the same time cancer is removed. In addition, if you’re having just one breast removed and reconstructed simultaneously, it’s unlikely that your breasts will “match” after all is said and done.

“Swelling, final healing, possible changes from radiation can all change the reconstructed breast,” Dr. Giese says. “When all has settled out, many times it is necessary to operate on the other breast to match the reconstructed side. Some women are just happy to be alive and have gotten through the initial surgical experience and do not proceed with surgery on the other breast. I strongly encourage women to move ahead with this step. Women simply feel much better when things are more symmetric…. Insurance in mandated in most states to cover this type of surgery,” she adds.

In Messina we left the ship around 11:00 am.? The first thing we did was to try a gelato since people rave about how great the ice cream is in Italy.? I have to admit that after trying several different flavors of gelato throughout the trip, I’m not sure what the hype is all about.? Gelato is good but it just tastes like ice cream to me.? But I had nothing to complain about since after all, not only was I eating ice cream, I was eating it in Sicily.

We walked with our ice cream until we came upon a very pretty church with a very unique bell tower.? Perched on several different levels of the tower were golden statues of various animals. At certain times during the day the animals would come to life and make their characteristic sounds (the lion would roar, the chicken would cluck, etc.) At the church we saw a tour bus which we decided would be our best bet for seeing Messina since we were still tired from walking around Pompei the previous day.? The tour was an hour and we got to see all the sights of the city.? The tour bus then dropped us off back at the church.

We really didn’t have a plan for the rest of the day so we decided to walk through the back streets of the city up to a cathedral I’d seen from the ship.? We weren’t sure exactly how to get there so it was an adventure making our way up there.? After about half an hour we found it.

It was on a hill overlooking all of Messina.? The view was spectacular. We could see the whole bay, including a full view of our ship.? The cathedral was closed to the public so we took some pictures and headed back down. After all that walking we decided to return to the ship and take a nap before dinner.
