With Dr. Calvert’s latest publication in the May issue of Plastic and Reconstructive Surgery, entitled “Reconstructive Rhinoplasty: Operative Revision of Patients with Previous Autologous Costol Cartliage Grafts” shows a sample of revision rhinoplasty patients who have had at least two previous rhinoplasty operations including one with rib cartilage has the possibility of nasal repair when skilled rhinoplasty surgeons use state-of-the-art techniques.

Why is this news such a break through? Previously patients who had been treated with rib cartilage grafts for secondary  rhinoplastery were considered to be out of options for surgical repair. However, following rib graft surgery not all procedures turned out to be as satisfactory to the patients and they often sought out additional surgery to improve both function and appearance.

All operations were planned carefully and performed using the highest manner possible. The patients were followed closely to insure that all which was possible to be done for them was used to achieve an optimal outcome. Most patients were treated with some form of re-grafting, either utilizing rib cartilage, ear cartilage or with Diced Cartilage and Deep Temporalis Fascia, a technique first reported by Dr. Rollin Daniel and Dr. Jay Calvert in the same journal in 2004.

The results were intriguing in that the rate of revision surgery after the initial surgery revision by Dr. Jay Calvert was 24%. The national average for revision among all rhinoplasty patients (primary or secondary) is 15%. Given the the difficult nature of these patient’s problems, that was not unpredictable. All patients had at least two operations and some had more than five operations. The pleasing outcome was after those 24% of patients had their second operations with Dr. Jay Calvert, their results were improved to the extent they did not require any additional surgery.
